Adrenocortical hypofunction characterized by hypotension, weight loss, anorexia, and weakness. The most common form is idiopathic Addison disease, mediated by autoimmune mechanisms. Autoantibodies specific to the adrenal cortex are specific diagnostic markers of this form.
Note: 21-Hydroxylase, a cytochrome P450 steroidogenic enzyme, is one of the major targets of adrenal autoantibodies in idiopathic Addison disease as well as in Addison disease in the context of autoimmune polyglandular syndromes (polyendocrinopathies, autoimmune). Hypofunction or failure of the adrenal gland may also be a manifestation of antiphospholipid syndrome due to thrombosis of the blood vessels of the adrenal glands.
